Register with us as a New Patient

We have an open list and welcome requests for registration from patients living in or moving to the practice area.

  • You can register by completing the relevant forms available from reception or print them off within the “Download forms ” section
  • You will be asked to complete a general health questionnaire and new patients requesting a health check will be seen by one of our practice nurses
  • Acceptance on to the list is as at the discretion of the Doctor
  • We will ask you for proof of identity. (Please refer to our Registration policy below)
  • If you are living outside our catchment area, you might like to try our affiliated practice at Woodhall Spa
  • Your medical records may take several weeks to arrive at the Practice.

We are happy to accept new patients who are ordinarily resident in the UK and living within our practice boundary.

Step One: Complete an online new patient registration form

The online registration form collects your details, a brief outline of your medical history and a record your preferences. This information will be added to your record and allow us to provide you with medical services.

Please complete one form for each family member.

Step Two: Provide evidence of current medication

We would like to know what medication you are currently taking. Please upload evidence of current medication with our online form.

If you would rather submit your application to register personally please download and complete the forms below. Please ensure that both forms are completed and that you complete all of the sections on the forms including your NHS number and full details of your previous doctor. This will enable us to retrieve your medical notes more quickly.  These registration forms will need to be completed for each member of your family that wishes to register with our practice.

The National Care Record Service (NCRS)

The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
